HLUB'09 Applications

This year we have 2 different applications; the HLUB'09 Attendee Application for the youth, and the  Adult Application for the youth chaperon.  Please print and use only the appropriate application. (Use the HLUB'09 Attendee Application for all children)

Discount: due to the lack of funding and financial resource, we do not have any scholarship for siblings, M/K and P/K.  The next best thing that the HLUB'09 team decided to do is to offer discounts for both the siblings and the M/K & P/K.  It's not much, but we hope that this will help a little bit.       Fill the application and read the "Release Statement" thoroughly, print and sign your name, have your parents or legal guardian sign the application.  Turn your application and the correct amount to the proper youth leaders in your church.  (Note: if you register together - you will likely room together.)

Adult Chaperon: download application here:  Application
     Fill the application and read the "Release Statement" thoroughly, print and sign your name.  (Please read the paragraph on your responsibility as a Chaperon on the application).  Submit application with the rest of your group.

Children 11 and under:  All children coming to HLUB'09 with the parents will need to sleep with the parents, there is no bed for them. All children MUST complete an individual application without the room fee, but with the food fees as stated here.  The following are the food rate for children:
1.  Children under 3                         Free of Charge
2.  Children between 3 and 7 pay      $  81.25
3.  Children 8 to 11                          $ 141.75 (regular price as adult)
Please staple children's application and correct fee with the parents' application.

Sorry, there was a mistake on the "HLUB'09 Individual Application" that caused confusion.  We apologize for the confusion.  We have revised the Individual Application.  Below is the explanation of the fees on the discount:

First Sibling Pay:                     $325.00
Second Sibling Pay:                $325.00
Third Sibling Pay:                    $300.00
Fourth Sibling on Pay...           $275.00
